which is available from the Venton Centre.

HOME FROM HOSPITAL SERVICE

If you have been in hospital and there is no one to help you when you get home, this service should be offered to you, along with the Social Service package. If this is not offered do ask at the Hospital before you are released, or you can contact: Zara O'Brien at AGE Concern on 01323 749854, or contact Derek or Kenneth for help.

The Home from Hospital Service - provides short-term practical help and support for anyone over the age of 55, who is about to leave hospital or has recently been discharged and who lives alone, or with someone who is physically unable to help. The Service covers the Eastbourne area, including Polegate, Westham and Pevensey. This Free Service provides volunteers, who are available to help with shopping and collecting prescriptions; providing information and assistance to help you obtain other services or equipment you need and may be able to accompany you to a one-off follow up medical appointment, or provide transport advice.

There is also a Befriending Service, which is also available from AGE Concern Eastbourne and you can get in touch through

Zara O'Brien.

The Befriending and Forget-me-not-Service - provides long-term support for older people who are living alone and do not have family or friends living nearby and who are lonely, frail or housebound. It offers regular support and friendship either by a weekly phone call or by a regular visitor to share a cup of tea and a chat.

Welfare Rights - This is a free service where they will guide you through the maze of available welfare benefits, which you might be entitled to, plus they will continue to help you until everything has been sorted out. You can either contact Andrea Edwards direct at andrea.edwards@ageconcerneastbourne.co.uk giving your details and a phone number or contact AGE Concern Eastbourne at the Venton Centre on 01323 638474

An Advocacy Service and an Information and Advice Service are also available at the Venton Centre on the above telephone number.

Information Sheets and Pamphlets

We are also collecting at "Eastbourne Rainbow" quite a few Information sheets and pamphlets from different people and organisations, so do ask Derek  if you need some help, if we do not have that information, then we shall do our best to obtain it for you or find someone who can help you with your problem.
